Sleeping on your stomach with your face directly in a pillow is a completely new way of sleeping and there are several Limitations related to this.
The sales package does not include a bed frame, head board or foot board.
Stable Platform:
The face rest scissor lift Proning Assembly needs to be placed on a stable platform. Some bed frames have slats which hold the mattress up. If your platform has slats you will need to place a 14 inch by 14 inch board over the slats where the face rest cavity goes.
Air Flow Constraints:
You cannot place the mattress head directly up against a wall, doing so will create a bowl effect which can trap the carbon dioxide you breathe out.
If the headboard you use has a solid backing where the mattress contacts you can also create a carbon dioxide trap.
Headboards come in so many styles it is hard to recommend one. It must have some kind of open architecture for air to flow around. Most Headboards once placed up against a wall leave 2 to 3 inches from the head of the mattress to the wall, this is enough space for the air to flow properly.
The wooden headboard you see in the picture has a nine inch opening. That matches the nine inch mattress with the wooden headboard opening so airflow is unaffected. At the same time the face rest proning assembly is over 3 inches away from the wall.

The Face Rest Scissors lift would not have a stable platform and during the night it will move as the sleeper moves.

This platform is a solid base and ideal for placing the Face Rest Scissor lift upon

Placing the Mattress on this type of platform may cause a carbon dioxide trap, causing the sleeper to re-breath their air.
